敬         告   1、申請(qǐng)人在填寫本申請(qǐng)書前，可登陸深圳市市場監(jiān)督管理局網(wǎng)站（http://www.szscjg.gov.cn/）參考企業(yè)注冊分類指南和各類示範(fàn)文本，也可到各注冊大廳領(lǐng)取。  2、申請(qǐng)人應(yīng)當(dāng)使用黑色鋼筆、簽字筆或毛筆工整填寫有關(guān)內(nèi)容和簽名，除簽名須由本人簽署外，其他內(nèi)容可以打印。 3、申請(qǐng)人提交的文件、證件應(yīng)當(dāng)規(guī)範(fàn)、清晰、潔淨(jìng)，應(yīng)當(dāng)使用A4紙。

Cautions:  1.Before completing this application form,the applicants can refer to the classification guides about enterprises register and all kinds of demonstrative texts from the website of Shenzhen Market Supervisory Authority(http://www.szscjg.gov.cn/).They also can obtain the reference from the registration hall.   2.The applicants shall use a black pen,sign pen or brush to fill in related contents and sign their names neatly. The application form can be printed except the signatures.   3.The submitted documents and certificates should be standardized, clear, clean,and in the  paper form of A4.

Shenzhen Market Supervisory Authority: 

According to laws, regulations and other relevant provisions, we apply for registration of the enterprise now, and we hope you can approve it. At the same time, we shall give the commitments as followings: we will guarantee that the submitted documents, certificates and related accessories are true, lawful and effective, and the copy is consistent with the original text. If any false documents and certificates occur and cause the bad consequences results, we shall bear all the corresponding legal responsibilities.
